Program Fact Sheet. (Formerly the "Administrative Checklist")
Faculty who are planning to lead a program with a travel component, either as part of an SYE or other on-campus course or program or as a course conducted entirely off-campus, must complete a Faculty-Led Travel Course Program Fact Sheet. This form will aid in establishing the program fee to be charged to the students. This fee must be reviewed and approved by the Department Chair, the appropriate School Dean, and the Business Affairs Controller prior to officially advertising the course and before registration for the course can begin. Travel Waiver Form
Everyone, including faculty members, students, relatives and others participants, traveling with and/or generally participating in an Otterbein University faculty-led travel course or program must sign a Travel Waiver Form (Word Document). Students will complete this and other required forms electronically and download them to their student file at www.otterbein.abroadoffice.net. Faculty should send a complete list of everyone in the group to the Center for International Education and Global Engagement no later than two weeks prior to the commencement of travel. This form can also be completed electronically by the students when they complete their profiles through CIEGE.
